Michael J. Sforzini, MD

Internal Medicine

Dr. Mike prides himself on the significant amount of time he spends not only getting to know his patient’s overall health concerns, but their health goals as well. As a young senior it is vital to have a doctor who prides himself on listening as much as doctoring. Dr. Mike is a doctor who prides himself on his bedside manner, warmth, and diagnostic abilities that will not only make you feel comfortable discussing your health, but also optimistic about meeting your health goals. He has extensive experience in working with Veterans and their families from the US Air Force Hospital to Veteran Facilities in the Nation. His knowledge ranges from elder to geriatric care to the issues all retirees and veterans face. He is proud to provide the best, most technologically advanced health care options you and your family deserve.

Nanjappa Chandramohan, MD, FACP, FACE, ECNU


Dr. ChandraMohan has been part of the community since 2002. He is both a clinician and a faculty member for the Ocala Regional Internal Medicine Residency program, teaching new internal medicine Residences how to be great physicians. Teaching has also enabled him to stay in tune with changes in the healthcare market. He has seen a broad perspective of the changes in patient care and has worked on building good relationships with primary care doctors in the community, as he treats their patients for endocrine related issues.


Carol Takis, MS

Physician Assistant

Carol received her MS Physician Assistant Certification from the University of Detroit, Mercy in 1999.  She came to Florida in 2005 and is a Board Certified Physician Assistant.  She can provide acute and chronic disease management, annual medicare wellness visits, general health physicals, DOT physicals, pre-op clearance and weight loss counseling.  She also has a background in respiratory therapy, which is how she started her career in 1985 at Sinai Hospital.  She received the Dean’s Scholar Award in 1995, 1996 and 1998. 


Shrikanth P.Y. Upadya, MD, FACC

Medical Director

Dr. Upadya became a part of the Villages community in 2006. He is a Board Certified Interventional Cardiologist, specializing in non-invasive and interventional management of cardiac and peripheral vascular disease. He is also Board Certified in Internal Medicine, Cardiology, Echocardiography, and Nuclear Cardiology. He is a member of the American College of Cardiology, and the American Society of Echocardiography. Dr. Upadya is also a member of the Central Florida Health Alliance. He has privileges at both Leesburg Regional Medical Center and The Villages Regional Hospital. Dr. Upadya also has consultant privileges at Citrus Memorial Hospital in Inverness, FL.
